Diabetes is one of the most prevalent chronic conditions in the United States, affecting over 37 million Americans — and millions more are living with undiagnosed pre-diabetes. At Blue Water Primary Care, our diabetes care program is designed to meet each patient exactly where they are, with a personalized, evidence-based approach to blood sugar management that produces real, measurable results.
Led by Dr. David A. Patel, MD, our diabetes program covers the full spectrum of care for Type 1 diabetes, Type 2 diabetes, and pre-diabetes. We begin with a thorough evaluation including HbA1c testing, fasting glucose, lipid panel, kidney function tests, and a review of your full medical and family history. From there, your provider builds a comprehensive care plan that addresses medication management, nutritional guidance, blood sugar monitoring targets, weight management, physical activity, and strategies for preventing diabetic complications.
We believe that patient education is just as important as medication. Our providers take the time to explain your condition clearly, ensure you understand how to monitor your blood sugar at home, and empower you with the knowledge and tools to take an active role in your own health management.
Regular follow-up visits are essential in diabetes care, and we schedule these at intervals appropriate to your level of control. We also coordinate with other specialists — including endocrinologists, ophthalmologists, and podiatrists — when needed to provide truly comprehensive, whole-body care. Most insurance plans cover diabetes management visits.
